Asphyxia (Hypoxic-Ischemic Encephalopathy)

Neonatal encephalopathy refers to altered behavior in the newborn characteristic of a disturbance in central nervous system functioning. The major etiology of neonatal encephalopathy is hypoxic-ischemic injury. Perinatal asphyxia with the resulting encephalopathy, is still one of the major problem in full term neonates. The overall mortality rate is approximately 20-30% & the frequency of neurodevelopmental sequela in surviving newborn is about 30-50%. The greatest risk of adverse outcome is seen in infants with sever fetal acidosis (PH

<6.7) & a base deficit > 25 mmol/L. So multiorgan failure & insult can occur. Brain monitoring at many centers using continuous or amplitude – integrated EEG provides important prognostic  information for standard care. MRI & MRS are good predictors of an adverse out come. The prognosis varies depending on the severity of the insult & the treatment.

Therapeutic hypothermia is neuroprotective & is standard therapy for (near) term neonates with sever perinatal asphyxia & encephalopathy which has reduced adverse outcome in infants with HIE from approximately 65% to 45%.
